What do people out there think?

Bust-enhancing herbal supplements, costing about $230 for an
eight-week supply, claim to increase a woman's breast size naturally, using
herbs like saw palmetto, hops, wild yam, and black cohosh, according to the June
2003 issue of Obstetrics & Gynecology. While they may be au naturel, that
doesn't necessarily mean they'll work.
"Buyer beware," says James Wells, MD,
president of the American Society of Plastic Surgery. "I don't think any of the
herbal products that are touted as breast enhancers show any promise. My
challenge is for them to show some scientific evidence."
No clinical trials
or animal studies on whether the alleged bust-enhancing supplements actually
increase breast size have been published, according to the Obstetrics &
Gynecology article. And "it is unlikely that any of these products, if they
contain what their labels say that they contain, would cause breast
enlargement."
Who believes this is true and who believes otherwise? Let me know!

Do you believe this?

Many breast-enhancing creams also claim to be all-natural,
but like herbal supplements, that doesn't mean you're going to get a favorable
result.
"Creams or lotions aren't going to work at all," says Dr. Wells, who
has a private plastic surgery practice in southern California.
According to
the FDA's web site, "Creams and lotions advertised as breast developers don't
work, either. Some contain the hormone estrogen. Estrogen can increase breast
size, but in order to be sold without a prescription these products must contain
such a small amount of the hormone that its effect is insignificant."

Are all of the ingredients safe for me to take? This is an all too important question that most women won't even think twice about before taking a herbal supplement. Most people figure that if it is on the market that it must be safe to take. This is far from reality. The makers of the pills are under no type of regulation whatsoever. Did you just read that? Other common names include black snakeroot, bugbane, bugwort, rattleroot, rattletop, rattleweed, and macrotys. Insects avoid it, which accounts for some of these common names. Black cohosh can cause stomach discomfort and headaches gastric complaints, heaviness in the legs, and weight problems. Women should also stay away from black cohosh if they are pregnant because it has a high rate of miscarage. People that have an allergy to asprin or other pain killing drugs should also steer clear of black cohosh because of problems.
Many of the breast pills use black cohosh as an ingredient though. They use it because it can have hormonal effects that can be a benefit to women going through hot flashes, or menopausal symptoms. It also helps to balance the hormonal system and then help in the increase of the breast tissue.

Software Enhancement Of Breast MRI Scans Help Radiologists Reduce False Positives

Using commercially available software to enhance breast scans done by magnetic resonance imaging (MRI) reduces the number of false positive identifications of malignant tumors and the subsequent need for biopsies, according to a new study.

Teresa Williams, M.D., and colleagues at the Seattle Cancer Care Alliance and the University of Washington Medical Center did a retrospective examination of 154 breast lesions deemed suspicious by radiologists that were only visible on MRI and that had been biopsied under MRI guidance. They compared the findings and recommendations made by radiologists at the time to new findings using computer-aided enhancement (CAE) software to enhance and evaluate the visible response to contrast agents absorbed by breast tissue.

False positives were reduced by 23 percent when CAE was set to its highest enhancement level, according to the study, which is published in the July edition of the journal Radiology. Williams was a medical resident in radiology at the Seattle Cancer Care Alliance (SCCA) when the research was done. She is now a fellow in pediatric radiology at Children's Hospital and Regional Medical Center in Seattle.

"In summary, our findings suggest that CAE has the potential to improve the discrimination of benign and malignant breast MRI lesions," the authors wrote. "We believe that CAE is useful as a tool to supplement the radiologist's subjective interpretation, but should not be relied upon exclusively to guide management."
"There are challenges associated with breast MRI and one is the time it takes to process and evaluate the many images acquired," said Constance Lehman, M.D., corresponding author and director of radiology at the SCCA. "Computer software programs such as the one evaluated in our study can assist us in interpreting breast MRI scans more easily. Our study suggests that the information provided may improve our ability to distinguish between benign and malignant lesions. Currently, MRI scans are used in addition to mammography when radiologists need a better view of tissue they suspect may be malignant.

MRI as an adjunct to mammography also is standard practice at the SCCA for women who are at high risk for breast cancer and to examine the other, or contralateral, breast of women who are newly diagnosed.
One particular challenge in breast MRI is the interpretation of the morphology and kinetic features -- the amount of contrast agent absorbed by breast tissue over time -- on multiple imaging series. Typically, a woman will receive one scan without contrast agent and two more after contrast has been administered. One key analysis function performed by CAE is automatic kinetic assessment.
"The detailed CAE lesion kinetic information differs substantially from that obtained by conventional manual placement of a region of interest," the authors wrote. This is because CAE generates detailed data for the entire lesion versus only a portion of the lesion that is highlighted by region-of-placement.

The lesions in the study had been identified and biopsied during 2001-2004 and came from 125 women ages 27-86. They were processed using CADstream™ 3.0, a CAE system developed by Confirma, Inc. of Kirkland, Wash. The presence of CAE threshold enhancement was sensitive for malignancy in 38 of the 41 malignant lesions examined using the software, according to the study. However, the software did not perform perfectly; it failed to confirm the malignancy of the three lesions. "Given the presence of three false-negative lesions, a finding deemed suspicious by the radiologist should be further evaluated regardless of the enhancement features determined by CAE," according to the study.

Williams said she advocates the use of CAE software analysis of MRI scans as an aid to radiologists' interpretations. "The software is already commercially available and it has shown it is useful in reducing the false positive rate of breast MRI," she said.
Summary of how CAE software works
From the journal paper: The CAE software incorporates three MRI series into its calculations: one pre-contrast T1 weighted series, one immediate post-contrast T1 weighted series and one delayed post-contrast T1 weighted series. The program compares pixel signal intensity values on the pre-contrast and immediate post-contrast series. If a pixel value increases above a user specified minimum enhancement threshold such as a 50 percent or 100 percent increase in enhancement, the pixel is said to meet "threshold enhancement."

Once a pixel has been identified as enhancing above the established threshold, the program then compares pixel signal intensity values on the immediate and delayed post-contrast series. If a pixel value on the delayed series decreases by more than 10 percent compared to the immediate post- contrast series, it is color-coded red, indicating a washout pattern of enhancement. If a pixel value increases by more than 10 percent it is color-coded blue, indicating persistent enhancement.

If a pixel value does not change in either direction by more than 10 percent it is color-coded green, for plateau enhancement. One of three colors (blue, green, or red) is applied to that pixel based on the delayed enhancement pattern.
This results in a color overlay map that is displayed on each MRI image, indicating regions of threshold enhancement.

Areas of threshold enhancement determined by the CAE software algorithm to be "connected" are summed and constitute a lesion. A synopsis of the kinetic enhancement details of the total connected area or lesion is automatically generated.

The physical aspect

In every surgical procedure, there are inherent risks. Most people are intimidated by the thought of 'going under the knife', even when it's medically necessary.
Cosmetic surgery, however, is just that -- cosmetic. It's not necessary to sustain health or treat disease, and therefore, taking the leap is even harder to justify.

During breast augmentation, the patient is put to sleep using general anaesthesia. Measuring the proper dosage can be very difficult for anaesthesiologists, because each individual reacts differently to the sleep-inducing drugs used. About five anaesthetic treatments per 1,000,000 result in death, usually caused by organ failure and suffocation.
Aside from the risks of surgery itself, there are many complications linked to breast enhancement. Implants have been known to shift, swell and wrinkle. Common complications include infection, loss or change of nipple sensitivity, breast pain and interference with breast feeding.

Moreover, practically all women who opt for breast enhancement procedures require several operations in their lifetimes -- implants should ideally be replaced once every decade. In drastic cases, as many as seven or eight procedures are conducted on a single patient.

Common reasons for follow-up surgery include ruptured implants, the asymmetric or lop-sided appearance of one or both breasts, and pain -- in some instances, an individual's body rejects the implants, and is not able to establish compatibility with them.
Also, there is no way to avoid scarring under the breasts. Though some women heal quickly, for others it is a terrible problem. A survey conducted in the United States revealed that about 7 percent of patients were unhappy with their enhancement procedures because of scarring.
Finally, there is the issue of quality to consider.

In the United States and Europe, where cosmetic surgery has been popular for over 20 years, there are medical boards and committees that govern facilities and doctors undertaking to perform such procedures. In India, on the other hand, the lack of regulations allows any quack to try his hand at cosmetic surgery. Outside of Mumbai and Delhi, there are very few properly trained and licensed specialists in breast augmentation.

When Do Breasts Start Growing?

Breasts can start growing as early as age 8 or as late as 13. Some girls' breasts grow slowly and others grow quickly. Some girls may feel like their breasts will never start growing. But it's just that girls start developing at different ages and different rates. One girl might have more developed breasts at 12, whereas her friend could be still flat as a board.
Breast development happens in stages.

A breast bud is like a small raised bump behind the nipple. After breast budding happens, the nipple and the circle of skin around the nipple (called the areola) get bigger and a little darker. Then the area around the nipple and areola starts to grow into a breast.
As breasts keep growing, they may be pointy for a while before becoming rounder and fuller. For some girls, one breast might be a little bigger than the other one. A girl's breasts may continue to grow during the teen years and even into her early twenties. Fully developed breasts come in all shapes and sizes.
Breast size gets a lot of attention and many girls may wonder how they can make their breasts grow faster or bigger. Despite what some magazine ads say, there isn't any magic cream or pill that can speed up the process or make a girl's breasts larger than they are. In fact, breast size is mostly determined by heredity and a girl's weight. So if a girl's mother has big breasts or small breasts, the girl can expect to have breasts of similar size. And a girl who has more body fat is more likely to have larger breasts.

Do implants last forever?

The average silicone implant lasts about 15 years, so it might need to be replaced in the future. Probblem with Breast Enlargement Surgery

After the operation there will be some discomfort on moving your arms, but this wears off after a week or two. Occasionally blood collects around the implant in the first 24 hours after surgery, and the surgeon may have to re-open the incision to remove the blood.

In the later stages of pregnancy, the mother’s hormones, which cross the placenta into the fetus, cause breast cells to organize into branching tube-like structures thus forming the milk ducts. In the final 8 weeks, lobules, (milk producing glands), mature and actually begin to secrete a liquid substance called colostrums. In both female and male newborns, swellings underneath the nipples and areolae can easily be felt and a clear liquid discharge (colostrums) can be seen. These represent the effect of the mother’s hormones and subside in the first few weeks of life.From infancy to just before puberty, there is no difference between the female and male breasts. With the beginning of female puberty, however, the release of estrogen, at first alone, and then in combination with progesterone when the ovaries functionally mature, cause the breasts to undergo dramatic changes which culminate in the fully mature form. Five easy breast enlargement exercises

Chest Press:Lying on the floor (or a bench), hold the dumbbells end to end just above the chest height. Be sure that your elbows are pointing out. Press the dumbbells straight up, extending your arms and hold for 15 seconds. Repeat for 10 times per day

Cobra Stretch:Lie facedown with your feet together, your toe pointed and you hands on the floor. Make sure that your palms are down just in front of your shoulders. If you feel any strain in your back, alter the pose so that you keep your elbows bent and your forearms on the floor.

Chest Lift:Lie face down on the floor with your hands under your chin. Lift your head, chest, and arms about 5 to 6 inches off the floor. Hold the position for 10 seconds. And Repeat 5-10 times per day.

Downward Dog:Position yourself on the floor on your hands and knees, your feet flexed. Press your hands and feet into the floor, raising your hips toward the ceiling. This is another dynamic-tension exercise that's perfect for the office, the car, or just about anywhere. Even simple and easy exercises like this increase your muscle tone and blood flow while helping build up the pectoral muscles.
Place your hands in front of you at about breastbone level.
Interlock your fingers. You can cup your hands and invert one, so they fit together comfortably. Alternately, you can link just your index and middle fingers like the links of a chain -- this way is much better if you have long nails.
Now, pull your two arms in opposite directions as hard as you can. Hold this pressure for five seconds.
Relax for five seconds. Now, repeat.

Lie face up on a bench. Your feet may be on the bench or flat on the floor, depending on what is most comfortable for you.
Grip a set of dumbbells and raise them to arm's length above the chest. Make sure your palms face forward.
Slowly lower the weight to the chest. Exhale as you raise the weight and inhale as the weight is lowered. Don't arch your lower back -- this is bad for your spine.

Inverted Push-Up

This exercise is a low-impact version of the traditional push-up. I recommend this exercise for those just starting out, or if you have very little upper-body strength. This is an exercise you can perform in the break room at work without working up much of a sweat.

Now lean against the wall, taking your weight on your hands. Your heels should be slightly lifted and your hands at shoulder width and height.

Lie face down on the floor and position your hands between 6 and 24 inches apart. (Putting your hands in different places changes the muscle group you're working. Experiment to see which position produces maximum results for you.)
Your palms should be down, in contact with the floor. Your legs and body should be straight and your head should be in line with the spine.
Press yourself upwards, fully extending your elbows and supporting the lower body on the toes. Raising yourself slowly maximizes the workout.
Lower down again, but do not allow your body to make contact with the floor. Lowering yourself as slowly as possible is best. Don't rush this part!
Repeat.
Alternately, for those of us with limited upper-body strength, this exercise can be performed on the knees rather than on the toes. Simply rest your knees on the ground. The rest of the exercise stays the same.

Choosing a doctor for breast implants...

Start by asking around. Do you have any friends or family who've had breast augmentation surgery? Go to them first and ask them about their experiences. Ask your family doctor if she can recommend anyone to perform the surgery.
Then contact the hospitals in your area. Ask which board-certified plastic surgeons have operating priveliges in the hospital. This privelige signifies official hospital approval for the surgeon, and is a sign of confidence.
As a last resort, check the yellow pages or online advertisements. And keep in mind that plastic surgery is a highly competitive field and there are many different surgeons and practices eager for your business.
Get some questions answered
Once you've gotten your list of possible surgeons, track down the following info to help narrow the list:
Check with the appropriate medical board in your state. Make sure the surgeon is licensed and has no pending cases against him.
The county clerk or circuit court in your area should be able to tell you whether the surgeon has any malpractice suits on record. Malpractice suits don't always mean a bad surgeon, but 3 or more cases within a 5-year period should be considered a warning sign.
Verify both credentials and training. Most doctors say they're "board-certified," but certified by which board? Is the board of certification even recognized by your state?
Make sure the surgeon has hospital admittance priveliges in case there is a complication during surgery.
Verify that the anesthesiologist is either a CRNA (certified registered nurse anesthetist) or a physician anesthesiologist.
Read the patient consent form and ask all your questions before signing it. Proper communication thoughout all the steps of the surgical process will result in greater satisfaction for both you and your doctor.

The American Board of Medical Specialities has a free online service (requires registration) that allows you to search based on your doctor's name for board certification status. Go here to verify all certifications.
Follow your instincts
Always meet face-to-face and trust your gut instincts. If there's something about the surgeon you don't like, keep looking. There are dozens of doctors perfectly willing and able to perform breast augmentation for you - there's no reason to settle.

Does weight loss mean smaller breasts?

It's a fact: most of us Americans could stand to lose some weight. So we eat better, we exercise, and maybe we shed a few pounds. But most women have a distinct concern: does losing weight mean an equivalent loss of breast tissue? If I get fit, will I need to buy smaller bras?
Weight loss generally affects the entire body at approximately the same rate. Places where excess fat is stored (butt, hips, thighs, etc.) lose a proportionally larger amount of fatty tissue. The breasts lose fatty tissue at approximately the same rate as the rest of the body.
But keep in mind that the breasts are not primarily composed of fatty tissue. The lobules and other tissue, which form the majority of the breast, will not shrink. Thus the reduction in breast size is generally minimal.
Note that everyone is different - and therefore loses weight differently. Your results may differ.

The cost of Implants

Breast implants are expensive. There's no way around it. Not only do you have to do your homework, find a good doctor, get on the doctor's waiting list, etc. - you also have to pay for it.
Depending on where you live, the cost of a complete breast implant procedure will range from $3500 to $10,000.

Here's the way the cost breaks down:
$1000-$3000 for the implants themselves
$1000 for the nurses and the anesthesiologist
The rest of the price you pay goes to the doctor. The greater the doctor's experience and demand, the higher the price will be.
You will pay more for a subpectoral implant
Add another $75 for prescription drugs and painkillers
And another $50 for post-op bras and other gear you'll need to speed your recovery.
The national average for the surgeon's fee only is about $3500 - not including associated expenses.
Keep in mind that this is surgery, so you'll be paying the hospital fee along with the labor and materials costs.
Some people fly to Mexico or South America for their breast implant surgery to keep costs down. There are a couple of drawbacks to this solution. First, countries outside the US have their own laws and enforcement regarding doctors and surgery. The FDA can't protect you in Venezuela. Second, you will not want to fly home right away -- you'll be in a lot of pain and on medication. So you should take into account the cost of room and board for yourself and someone to look after you for at least a few days after the surgery.
Financing breast implant surgery
There are some companies that will help you finance breast implant surgery, or offer a loan against the cost of surgery. Your plastic surgeon may offer you information about financing. Keep in mind that financing is done by a completely separate entity.
Also, pay attention to interest rates. Medical financing is sometimes offered at only relatively high rates.
Keep the cost in mind when considering breast implants. Also keep in mind the likelihood of complications a few years down the road - you'll have to pay for those procedures, too. Yes, it is expensive - but somehow, thousands of women are able to come up with the money every year.

Dark side of breast implants


Nearly everyone has heard one or more horror stories about breast implants rupturing, or shifting, or somehow going wrong. The truth is, inserting breast implants is major surgery and should be treated as such. Here are a few of the more common ways in which breast augmentation surgery can go wrong:
Anesthesia: Local anesthesia is extremely safe. Twilight or full anesthesia are riskier. Every year, Americans die from mistakes made by anesthetists. Although anesthesists are highly trained professionals, mistakes can still be made.
Infection: Infection is a risk of any surgery, no matter how minor. An infection during breast implant surgery would almost certainly necessitate the immediate removal of the implant.
Recovery time: You will not be able to resume your normal activities for some time after surgery. Even after you are up and around, you will need to wear specially padded bras and take painkillers to reduce the pain and discomfort of the breast implants.
Rupturing implants: For the rest of your life, there is a risk, however small, that the breast implants will rupture. This would be a medical emergency, potentially life-threatening.
Difficulty of mammography: Breast implants make mammography, and therefore detection of breast abnormalities and tumors, more difficult. The level of difficulty is decreased if you opt for a submuscular breast implant.

But how safe are they, really?

The Plus Side
Breast implants for breast augmentation is by far the most dramatic and effective way to get bigger breasts. There's absolutely no other way to create the same kind of dramatic changes that breast implants can make. A woman can go from an AA to a C cup within a few hours. And the latest generation of implants feel even more natural than ever before. Surgeons are able to employ techniques that leave very small scars.
There are alternatives that are less expensive, but are also less immediate and less dramatic, including: breast enlargement pills, breast enlargement creams, breast enlargement exercises and breast pumps.

Dermal Fillers

Breast implant technology is changing fast, and soon implants themselves may be a thing of the past. Right now there is a breast augmentation solution available that is narly instant and doesn't require surgery or a long recovery period.
Have you heard of dermal fillers? These materials are gel-like compounds that can be injected into the body. They increase the volume of the area they're injected into and are both stable and harmless in the body. Restylane and Hylaform are types of dermal fillers
Swedish scientists have developed a dermal filler called Macrolane (made by the same company that makes Restylane) specifically for breast augmentation. Made of stabilized hyaluronic acid (similarly to Hylaform), Macrolane is a soft gel-like substance that can be used to correct misshapen breast implants, contour and even add bulk to breasts. This procedure is not permenant - all dermal fillers are eventually absorbed and metabolized by the body. One round of Macrolane breast augmentation should last from 1-2 years.
Macrolane is composed of hyaluronic acid but in larger particles than those found in Restylane. This makes it more suitable for larger jobs, and also means the body absorbs it more slowly.
Using dermal fillers for breast augmentation is an out-patient procedure. After applying a local anesthetic, the doctor injects the chosen substance into the patient and that's pretty much it. The entire procedure lasts less than two hours. Because it's non-surgical, there are none of the risks of full anesthesia, nor of complications arising from surgery. Recovery time is almost zero. In short, nearly all of the features of Macrolane make this superior choice over surgery for breast augmentation.
Here's the bad news - Macrolane is not currently available in the United States. The Macrolane breast augmentation procedure is being done in France, where it seems to be meeting with general approval.
Maybe soon we will have instant-gratification non-surgical breast augmentation without the discomfort and expense of surgery.

Are breast pumps a viable breast enlargement solution?

Temporary breast pumps are not a viable breast enlargement solution. In fact, overuse of this type of breast pump may cause broken blood vessels and other health issues.
The Brava system certainly does work. But there are some significant drawbacks:
It's expensive. At about $2500, cost is a significant factor for most people.
It works slowly. The Brava system needs to be used for about 6 months for maximum results.
It should be worn 12 hours per day. Even though the Brava breast pump is fairly low-profile, rather like a padded bra, many people find this recommendation difficult to follow.
Gains are modest. On average, women gain from 1/4 to 1/2 cup even after following all the guidelines faithfully.

How breast pumps work


Breast pumps work through the physiological process known as tissue expansion. Basically, if you apply a gentle pressure to stretch skin or other soft tissues, they slowly stretch. If you've ever seen a person with oversized ear plugs or a native tribesman with a large ornament in his lip, you've seen tissue expansion at work.
By applying relatively gentle vacuum pressure to the breasts, breast pumps encourage the process of tissue expansion. Over time, this leads to a modest but permanent increase in breast size. This applies to the Brava breast pump, which is to be worn approximately 12 hours per day.
Another, temporary, type of breast pump is also available (usually for less than $30). These manual breast pumps consist of a plastic dome and a small hand pump. By applying a greater vacuum force to the breast, this type of pump encourages blood to engorge the breast and nipple. This increases breast sensitivity and also can increase size slightly, but these gains are temporary, disappearing within two hours.

The Risks

The risks associated with breast augmentation surgery:
§ Repeat surgery. You may need more surgery down the road, either to replace the implants or to remove them altogether. In clinical trials studying the safety and effectiveness of saline-filled breast implants, as many as one in four women required a second operation within five years of the initial surgery.
§ Rupture. Implants can rupture, causing fluid to leak into your breast and surrounding tissue. This can happen as a result of a blunt-force injury — if you're thrown against the steering wheel in a car accident, for example — or from tiny cracks in the implant shell that can occur over time.
If you have a saline-filled implant, a rupture will cause your implant to lose its original size or shape. If you have a silicone gel-filled implant, a rupture may not be as obvious. Your doctor may use occasional imaging tests, such as magnetic resonance imaging, to screen for rupture.
§ Deflation. A rupture or a slow leak can cause an implant to collapse, deflating the size of your breast. This may result in breasts that are noticeably different in size or in a change to the cosmetic appearance of your breast, such as sagging or wrinkling.
§ Capsular contracture. Fibrous scar tissue forms a capsule around your breast implant. The scar tissue builds up over time and constricts your implant — a painful and potentially disfiguring condition. Surgery is usually necessary to correct capsular contracture.
§ Infection. Breast augmentation surgery can lead to infection. Medications may help, but antibiotics aren't always successful in treating infections of this type. Removing your breast implants may be necessary if you develop a severe infection. You may have to wait six months to a year after implant removal before you can get new implants placed.
§ Hematoma. Blood and other fluids can pool around the implant, causing pain, infection or other problems. If you develop a hematoma, you might need to go back into the operating room so that your doctor can find the cause of the bleeding.
§ Pain. You could experience significant pain after surgery — more than what's considered normal — which indicates a bigger problem, such as implant rupture or capsular contracture. To remedy the problem, you may need surgery to remove or replace the breast implants.
Other possible risks include dissatisfaction with the results of your surgery. You might experience changes in the sensation of your breasts and nipples, or you might be able to feel the implant beneath the surface of your breast tissue.

Breast enlargement creams

Breast enlargement creams can provide additional gains when paired with breast enlargement pills. The principles behind breast enlargement creams are similar: they deliver plant-based estrogen-like compounds to the breasts, thus stimulating the estrogen receptor sites in the mammary glands to produce additional breast tissue.
Because the ingredients are topically delivered, they can be more "targeted" than a pill, which by definition has to be digested and circulated through the entire body before taking effect.
Additional ingredients may be added to breast enlargement cream to improve circulation. L-arginine has been shown to boost circulation by stimulating the body's production of nitric oxide.
